Quick Overview.

SS-31 (also known as Elamipretide, Bendavia, or Forzinity) is a synthetic tetrapeptide (four amino acids) specifically designed to target and repair mitochondria. While MOTS-c acts like the gas pedal for your cellular engines, SS-31 acts like the mechanic that fixes the engine itself.[1]

Inside your mitochondria, there is a specialized lipid (fat) called cardiolipin. It acts like the oil that keeps the engine running smoothly and maintains the structure of the inner mitochondrial membrane. As you age, or if you suffer from a chronic illness, cardiolipin becomes oxidized and damaged. The engine starts sputtering, leaking toxic exhaust (reactive oxygen species, or ROS) into your body. SS-31 dives directly into the mitochondria, binds to the cardiolipin, repairs the membrane, and stops the toxic exhaust, allowing the engine to produce clean energy (ATP) again.[2]

  • Primary Use Case: Repairing mitochondrial dysfunction, reversing chronic fatigue, and treating age-related energy decline.
  • Mechanism: Selectively binds to cardiolipin on the inner mitochondrial membrane, stabilizing its structure and restoring the efficiency of the electron transport chain.[2][3]
  • Who it is for: Older individuals, those recovering from chronic illness (Lyme, Long COVID, mold toxicity), or anyone suffering from severe, unexplained fatigue.
  • Who it is NOT for: Young, healthy individuals with perfectly functioning mitochondria (you will likely feel absolutely nothing from taking it).

02 · The Protocol & Usage Guide

The Protocol & Usage Guide.

confidence_tier: well-established

SS-31 is generally very well tolerated, but it is incredibly expensive. Because the daily dose is relatively high and it needs to be taken daily for weeks to see results, a full cycle can cost thousands of dollars.

Standard Dosing

Note: Clinical trials often use much higher doses (up to 40mg/day), but the biohacking community has found a "sweet spot" for cost-to-benefit ratio.

Experience LevelDoseFrequencyTiming
Standard Protocol4.0 mg1x dailyMorning
Aggressive Protocol8.0 mg1x dailyMorning

Cycle Length

  • Standard Cycle: 4 to 8 weeks.
  • Discontinuation: Can be stopped abruptly.

Reconstitution Math

Note: SS-31 is often sold in larger 50mg vials due to the high daily dose.

  • Vial Size: 50 mg (50,000 mcg)
  • Bacteriostatic Water Added: 2.5 mL
  • Concentration: 20 mg per mL

Contraindications

  • Absolute: None explicitly known, though pregnant or breastfeeding women should avoid use due to a lack of safety data.

Drug Interactions

  • No significant negative drug interactions have been reported in clinical literature.

StackGoalRationale
SS-31 + MOTS-cThe Mitochondrial OverhaulThe Gold Standard. Think of your mitochondria like a car engine. You don't want to press the gas pedal to the floor (MOTS-c) if the engine is leaking oil. You want to fix the oil leak first (SS-31), and then press the gas pedal. Many users run 4 weeks of SS-31, followed immediately by 4 weeks of MOTS-c.
SS-31 + NAD+ (Injectable)Extreme Energy RestorationSS-31 repairs the physical structure of the mitochondria, while NAD+ provides the raw fuel required for the Krebs cycle to produce ATP.

Body Composition & Training Guide.

SS-31 is not a direct fat burner or muscle builder.

  • Exercise Tolerance: Its primary benefit for athletes is restoring exercise tolerance in individuals who have suffered from overtraining syndrome or chronic illness. By fixing the mitochondria, the body can once again produce enough energy to sustain a workout without crashing afterward.[8]
  • Tracking Progress: Track subjective feelings of "brain fog," baseline energy levels upon waking, and the ability to recover from physical exertion.
07 · Storage, Handling & Accessibility

Storage, Handling & Accessibility.

confidence_tier: well-established

  • Unreconstituted (Lyophilized Powder): Store in the freezer (-20°C) for up to 24 months.
  • Reconstituted (Liquid): Must be stored in the refrigerator (2-8°C). Good for 30 days.
  • WADA Status: Not explicitly banned, as it is not an anabolic agent or hormone secretagogue.

SS-31 does not typically alter standard blood markers. If using it to treat a specific condition, monitor the markers relevant to that condition (e.g., inflammatory markers, lipid panels).

Mechanism of Action

SS-31 (D-Arg-dimethylTyr-Lys-Phe-NH2) is a cell-permeable, mitochondria-targeted tetrapeptide.

  1. Cardiolipin Binding: SS-31 selectively targets and binds to cardiolipin (CL), a unique phospholipid found almost exclusively in the inner mitochondrial membrane (IMM). CL is essential for the optimal function of numerous enzymes involved in mitochondrial energy metabolism, including the electron transport chain (ETC) complexes.[2]
  2. Structural Stabilization: By binding to CL, SS-31 stabilizes the IMM structure, preventing the leakage of electrons from the ETC. This prevents the formation of reactive oxygen species (ROS) at the source, rather than just scavenging them after they are formed (which is how traditional antioxidants like Vitamin C work).[3]
  3. Cytochrome c Optimization: SS-31 protects cytochrome c (a key protein in the ETC) from peroxidase activity, ensuring efficient electron transfer and maximizing ATP production, even under ischemic or highly stressed conditions.[4]

Clinical Trial Summary

  • Barth Syndrome: In September 2025, the US FDA granted accelerated approval to Elamipretide (under the brand name Forzinity) as the first treatment for Barth syndrome, a rare genetic disorder characterized by abnormal cardiolipin remodeling and severe mitochondrial dysfunction.[5]
  • Primary Mitochondrial Myopathy (PMM): While earlier Phase 3 trials (MMPOWER-3) for PMM did not meet their primary endpoints for improving the 6-minute walk test, post-hoc analyses and ongoing studies continue to explore its efficacy in specific mitochondrial disease subsets.[6]
  • Heart Failure: Clinical trials have demonstrated that a single infusion of Elamipretide is safe and well-tolerated in patients with heart failure, resulting in favorable changes in left ventricular volumes.[7]

Frequently Asked Questions (FAQ).

Q: I'm a healthy 25-year-old athlete. Will SS-31 give me more energy? A: Probably not. If your mitochondria are already healthy and your cardiolipin is intact, SS-31 has nothing to "fix." It is not a stimulant. It is a repair tool for damaged or aging mitochondria.

Q: Why is it called SS-31, Elamipretide, and Bendavia? Are they the same thing? A: Yes, they are the exact same molecule. SS-31 is the original research name (named after the inventors Szeto and Schiller). Elamipretide is the generic pharmaceutical name. Bendavia and Forzinity are brand names.

Q: Can I take it orally? A: No. As a peptide, it will be destroyed by your stomach acid. It must be injected subcutaneously.
